How to register

We are registering new patients and welcome requests for registration from patients living in the practice area, approximately within a mile from the surgery. This boundary can be viewed via this link:

We are unable to accept patients who live outside our practice area.

To register please bring along your NHS medical card or NHS number and complete a registration form available from reception or via this link:

If you are unable to find your NHS card, please contact your previous surgery for your NHS number. The NHS number is vital to your registration with the practice. Please bring proof of address and ID with you. If you do not have an NHS number please bring proof of residency in the UK and your National Insurance Number.

Registrations have to be done in person. If there are access issues please contact the reception team. Registrations take about 10-15 minutes and can be done Monday to Friday  from 8.45 am up to 6.30pm. Busy times at reception  are 8.45am to 11am and 5.30-6.30pm and this may lead to some delays.

We would like all new patients undertake a health check and you will be asked to make an appointment to see the doctor. Without the new patient check we are unable to complete the registration process.
