How to register

We have an open list and welcome requests for registration from patients living in the practice area.

We do not accept those living out of the catchment area.

To register with the practice we need:

  • Your NHS number ( you can access this online or from your NHS card)
  • A completed registration form (see below for options)

You can download and print a registration form or you can complete our online form.

Please bring proof of address and ID with you to the Practice to complete your registration.

If you do not have an NHS number please bring proof of residency in the UK and your National Insurance Number.

Registrations can be to be done in person. If there are access issues please contact the reception team. Please note that our practice is located in the basement, with no lift access. There are other local practices that have disabled access.

Registrations take about 10-15 minutes but take 5 days to become live on our system. These can be done Monday to Friday  from 8.45 am up to 6.30pm. Busy times at reception  are 8.45am to 11am and 5.30-6.30pm and this may lead to some delays.

We would like all new patients undertake a health check and you will be asked to make an appointment to see a clinician. Without the new patient check we are unable to complete the registration process.

If you move out of the practice, area you will asked to re-register locally.  This is due our contractual arrangement with NHS England. In addition many community services and mental health services are based on the fact that you are a Camden resident and live locally to the practice.
